Medical Review
Dr. Park will review your vaccination records, assess your medical history, and perform a comprehensive physical exam.

Lab Testing
Age-specific lab tests for syphilis, gonorrhea, and tuberculosis will be ordered. These tests can be completed in-office or at a Quest Diagnostics location.

Vaccination Assessment
Dr. Park will confirm your vaccination status and inform you of any additional vaccinations required, such as:

Form I-693 Completion
Once lab results are received (typically within one week), you’ll receive your signed Form I-693 in a sealed envelope for USCIS submission. A copy will also be provided for your records.
Important: Do not break the seal on the envelope, as USCIS will reject it.

Immigration Medical Exam
The immigration medical exam includes:

Physical exam, lab tests, chest x-ray, and some vaccinations (Flu & Tdap)

Additional Costs:

We offer the Flu and Tdap vaccines in our office. Other required vaccinations can be obtained at local pharmacies.

If you need a chest x-ray (for a positive TB test), it will be performed at Granada Hills Radiology for an additional fee.
Payment Information
We accept cash and credit cards. A deposit is required at the time of the appointment. Please note that we do not accept insurance for the immigration exam, lab tests, vaccines, or chest x-rays.
